Transit stopovers lasting less than 24 hours (where you do not leave the airport) do not need to be included, except for the USA. If you are traveling to the USA at any point during your journey, you must include it as one of your destinations.
If your stopover involves you leaving the airport for an overnight stay, you should add this country destintation to your policy.
